Job summary

Havenwyck Hospital, a UHS facility in Auburn Hills, MI, is seeking a licensed Social Worker to join the inpatient team. The role covers social services for inpatient and day hospital patients, discharge planning, and family and group therapies as part of a multidisciplinary team led by a psychiatrist.

Qualifications

• Master of Social Work degree from a CSWE-accredited program, and/or a MA degree required.
• Current License in Michigan as LMSW, LLMSW, LPC, or LLC required.

Responsibilities

• Participates as a professional member of a multidisciplinary treatment team led by the psychiatrist.
• Facilitates the therapeutic milieu on the treatment unit.
• Performs social work interventions as directed by the team and treating psychiatrist.
• Obtains and assesses social history information and reports to treatment teams.
• Performs discharge planning and case management from admission.
• Provides group, individual, family, substance abuse, and couples therapy as deemed appropriate.
• Acts as liaison for hospitalized patients with community agencies and resources.
• Attends treatment team conferences and completes treatment plans.
• Documents in the medical record.

Education

MSW degree

Michigan LMSW/LMSW or LPC/LLC license
