Job summary

Havenwyck Hospital in Auburn Hills, Michigan, seeks an Outpatient Clinician (Clinical Social Worker) to join the multidisciplinary team delivering social services to patients in outpatient settings.

You will conduct assessments, develop treatment plans, provide individual, family and group therapy, coordinate supports, and participate in discharge planning and program improvement activities within a compassionate care environment.

Qualifications

• Master of Social Work or Master of Arts in Counseling required.
• Current Michigan LMSW or LPC license required.
• Good standing and eligible for internal transfer.

Responsibilities

• Participates as an integral member of a multidisciplinary treatment team.
• Engages in patient assessment, treatment planning, and documentation.
• Provides individual, family, and group therapy and discharge planning.
• Engages in supports coordination and collateral contact as indicated.
• Works a schedule meeting program needs and attends team meetings.
• Participates in Performance Improvement activities.
