Job summary

Havenwyck Hospital in Auburn Hills, MI is seeking a licensed Social Worker to join our inpatient team on a PRN basis. The Social Worker provides clinical social services to inpatient and day hospital patients, participating in a multidisciplinary team led by the psychiatrist.

Responsibilities include discharge planning, individual and group therapy, liaison with community resources, and documenting in the medical record.

Qualifications

• MSW degree or MA in a related field.
• Current LMSW, LLMSW, LPC, or LLC license in Michigan required.
• Experience on an inpatient psychiatric unit preferred.
• Employee must be in good standing for internal transfers.

Responsibilities

• Participates as a professional member of a multidisciplinary treatment team.
• Facilitates the therapeutic milieu on the unit.
• Performs social work interventions as directed by the team and psychiatrist.
• Obtains and reports social history information to treatment teams.
• Performs discharge planning/case management from admission.
• Provides group, individual, family, and other therapies as appropriate.
• Acts as liaison to community agencies and resources.
• Attends treatment team conferences and completes plans.
• Documents in the medical record.
