Qualifications

• Masters Social Work is required.
• Licensed Specialist Clinical Social Worker (LSCSW) or Licensed Masters Social Worker (LMSW) certification is required.

Responsibilities

• Accepts responsibility for achievement of optimal outcomes within scope and follows policies and procedures.
• Performs psychosocial assessments of patient/family situations to facilitate return to the community.
• Identifies information to assess patient’s psychosocial needs and discharge plan.
• Participates in interdisciplinary team meetings and family conferences to determine needs.
• Documents psychosocial assessments and discharge planning in patient records.
