Job summary

MedStar Health is seeking an LMSW/LMGW Inpatient Case Manager to join the Case Management Team in Maryland. The role focuses on developing individualized discharge plans, coordinating care from pre-admission to post-discharge, and promoting efficient use of resources to achieve optimal patient outcomes.

The ideal candidate will have a Master’s in social work, be licensed as a Social Worker in DC or MD, and have experience in hospital settings; CCM certification is preferred.

Qualifications

• Master's degree in social work from an accredited school.
• Valid Social Worker license in DC or MD depending on work location.
• Experience in hospital social work or care/case management preferred.
• CCM - Certified Case Manager preferred.

Responsibilities

• Collaborate with multidisciplinary team to develop and coordinate the plan of care with long- and short-term goals.
• Communicate with caregivers and case management triad about discharge planning and payor concerns.
• Maintain documentation of case management activities and ensure confidentiality.
• Develop discharge plans and coordinate community resources to meet patient needs.
• Organize workloads and prioritize based on medical plans and policy.
