Job summary

Atrium Health seeks a Master’s level social worker to provide case management and psychosocial support during discharge planning and care coordination.

You will interview patients and families, complete psychosocial assessments, develop treatment plans, coordinate referrals, and advocate for patient needs while staying current with payer practices; CSWE-accredited MSW and licensure are preferred.

Qualifications

• Master’s degree in Social Work from CSWE-accredited program.
• Licensure or provisional status desirable.
• ACM-SW certification preferred.
• Experience in healthcare social work is desirable.
• Knowledge of payer and reimbursement practices.

Responsibilities

• Identifies patients needing Case Management based on discharge screening.
• Assesses data by interview and evaluation of patient needs.
• Completes psychosocial assessments per standards to identify needs.
• Develops and implements psychosocial treatment plans using various modalities.
• Initiates referrals tailored to individual patient needs.
• Maintains knowledge of federal, state, local regulations.
• Advocates for unmet patient needs.
• Participates in discharge planning with the multidisciplinary team.
• Documents in the EMR per reporting standards.
• Guides patients through post-discharge options.
