JPS Health Network seeks a Social Worker II – Inpatient to coordinate discharge planning and social work services on inpatient units, collaborating with the care team to optimize utilization, finances, and patient satisfaction.

The role requires a Master’s in Social Work and LMSW/LCSW licensure with 1–3+ years in inpatient acute care or behavioral health, performing assessments, referrals, and individualized discharge planning.

Qualifications

• Master’s in Social Work from an accredited school of Social Work by the CSWE.
• 1–3 plus years social work experience in inpatient acute care, behavioral health, or related settings.
• Experience with discharge planning, case management, and navigating social services.

Responsibilities

• Conducts comprehensive psychosocial, functional, financial, healthcare support systems, and discharge planning assessments.
• Interviews patients and families to assess needs and develop individualized discharge plans.
• Identifies eligibility for social programs and coordinates referrals.
• Develops discharge plans based on diagnosis, resources, and psychosocial factors.
• Educates patients and families about post‑acute options and ensures choices are included in plans.
• Participates in interdisciplinary rounds to coordinate care.
• Provides counseling related to treatment decisions and autonomy.
• Assists with guardianship applications and APS/CPS coordination as needed.
• Identifies barriers for uninsured/undocumented patients and connects to resources.
• Documents activities in EMR per policy.
