Job summary

American Traveler is recruiting an experienced RN Case Manager for an inpatient care management role focused on acute hospital discharge planning. Must hold NJ or compact RN licensure and BSN, with CCMC certification preferred.

The position features a 37.5-hour week on a day shift (8:00 AM–4:00 PM) with rotating weekends and holiday coverage. Prior discharge planning and subacute care experience are required, using Epic EMR to coordinate transitions.

Qualifications

• Active RN license (NJ or Compact)
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience
• Hospital experience including discharge planning and rehab facility placement
• Experience with subacute patients
• In-Patient Case Management experience required
• Experience with Managed Care preferred

Responsibilities

• Hospital-based inpatient Case Management/Utilization Review
• Day shift 8:00 AM to 4:00 PM, 37.5 hours per week
• Rotating weekends every 4th or 5th weekend
• Holiday coverage for one summer and one winter holiday
• Floating as needed to a sister hospital nearby
• No on-call responsibilities
• Uses Epic EMR for charting
• Coordinate patients through acute care hospitalization and manage discharge planning and transition
• Perform utilization management and quality screenings
• Accepts local candidates at a lower rate
