American Traveler is seeking an RN Case Manager for an inpatient Care Management role in a CA hospital. The position requires CA RN licensure and RN Case Management certification, with BLS, and inpatient CM experience.

Day shift, 40 hours per week, 8:00 AM to 4:30 PM, Sunday-Thursday. The role involves coordinating care from pre-admission through post-discharge, with focus on complex patients and regulatory compliance.

Qualifications

• Active CA RN license required.
• RN Case Management (CM) certification required.
• BLS certification required.
• Inpatient case management experience required.
• Cerner and Epic experience noted in employment history if applicable.

Responsibilities

• Unit: RevCycle - Care Management within an acute care hospital setting.
• Day shift, 8:00 AM-4:30 PM, Sunday-Thursday, 40 hours per week.
• No on-call requirements.
• Handles a heavy caseload including complex patients, frequent ED utilizers, chronic pain, substance abuse, and homeless patient populations.
• Coordinates continuum of care from pre-admission through post discharge.
• Collaborates with acute care case managers and coordinates transfers to tertiary centers.
• Acts as liaison between patient, family, physicians, multidisciplinary team, and healthcare benefactors.
• Monitors appropriateness and timeliness of interdisciplinary care plans and resource utilization.
• Provides oversight and data collection for regulatory and accreditation compliance.
• Conducts focus study reviews for frequent ED visitors.
• Candidates with more than 5 days of requested RTO will be automatically declined.
• Candidates with any prior adverse licensure action will not be considered.
• No radius rule in place; local candidates accepted at the same rate as travel candidates.
• Travelers may only be submitted to one hospital within the Adventist Health system at a time.
