Job summary

A-Line Staffing Solutions is seeking an experienced Registered Nurse to work in an acute care hospital as an Inpatient Case Manager in Summit, NJ. You will coordinate patients during hospitalization and drive discharge planning, transitions of care, and utilization management to support optimal patient outcomes.

Required BSN and active RN license with strong knowledge of managed care. 2+ years of acute care and inpatient CM experience are preferred to join our multidisciplinary team.

Qualifications

• Inpatient case management experience is required.
• RN license is required; BSN is required.
• 2 years of acute care experience strongly preferred.
• Experience in case management and managed care.
• Strong understanding of discharge planning and transitions of care.

Responsibilities

• Coordinate patients throughout the acute care hospitalization.
• Manage all aspects of the discharge planning and transition process.
• Perform utilization management and quality screenings.
• Coordinate care and services to support appropriate patient outcomes and transitions.
