Job summary

American Traveler is hiring a Registered Nurse Case Manager for an acute care hospital in New Jersey. The role requires an active NJ (or compact) RN license and at least two years of inpatient discharge planning experience.

The ideal candidate will have strong hospital-based care management skills, experience with subacute patients, and familiarity with utilization management. EPIC charting is used; shifts are day hours with rotating weekends.

Qualifications

• Active NJ or compact RN license
• BSN required
• CCMC certification preferred
• Minimum 2 years of acute inpatient hospital discharge planning experience
• Hospital experience in discharge planning and placement in rehabilitation facilities
• Experience with subacute patients
• Case Management and Managed Care experience
• In-Patient Case Management experience

Responsibilities

• Hospital-based Care Management unit
• Coordinates patients through acute care hospitalization
• Manages all aspects of discharge planning and transition process
• Performs utilization management and quality screenings
• Works with subacute patients and discharge placement to rehabilitation facilities
• AM shift, 8:00 AM to 4:00 PM, 37.5 hours per week
• Rotating weekends required every 4th or 5th weekend
• One summer and one winter holiday coverage requiredMay float to a sister hospital as needed
• No on-call requirements
• Uses EPIC for charting
• Coordinates comprehensive discharge planning and transition of care for acute hospital patients
• Performs utilization review and quality screening as part of daily responsibilities
• Local candidates accepted
• Travelers may extend but are limited to one calendar year on assignment before requiring a break
