Job summary

Sentara Williamsburg Regional Medical Center in Williamsburg, VA is seeking a Registered Nurse - Inpatient Case Manager. You will coordinate comprehensive care management and facilitate transitions to appropriate levels of care for hospitalized patients.

The role emphasizes collaboration with the care team and the patient/caregiver to develop and implement a holistic care plan addressing physical, mental, and psychosocial needs.

Qualifications

• 12 months nursing experience required; critical care experience preferred.
• BLS within 90 days of hire.
• Associates of nursing Required; BSN is preferred.
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
• 1-year acute care case management experience preferred.
• Experience with community resources preferred

Responsibilities

• Provide and facilitate comprehensive care management across the patient care continuum.
• Support hospital patients needing case management, focusing on transitioning to appropriate level of care.
• Develop and implement care plans addressing physical, mental, and psychosocial needs.
