Job summary

Sentara Williamsburg Regional Medical Center in Williamsburg, VA is seeking a Registered Nurse - Inpatient Case Manager to lead comprehensive care management for hospitalized patients, focusing on transition to the appropriate level of care and LOS optimization.

The role requires 12 months of nursing experience (critical care preferred), BLS within 90 days, and case management certification within one year.

Qualifications

• 12 months nursing experience required; critical care experience preferred.
• BLS within 90 days of hire.
• Associates of nursing required; BSN is preferred.
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
• 1-year acute care case management experience preferred.
• Experience with community resources preferred.

Responsibilities

• Provide and facilitate comprehensive care management and quality outcomes across the healthcare continuum.
• Focus on patient transition to appropriate level of care and clinical appropriateness and LOS.
• Support hospitalized patients identified for comprehensive case management.
• Promote effective utilization and monitoring of health services; collaborate with the care team, patient and caregiver.
• Develop and implement a comprehensive care plan addressing physical, mental, and psychosocial needs of the inpatient/caregiver.
