Qualifications

• 12 months Nursing experience required; critical care experience preferred.
• BLS within 90 days of hire.
• Associates of nursing Required; BSN is preferred.
• Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
• 1-year acute care case management experience preferred.
• Experience with community resources preferred

Responsibilities

• Provide and facilitate comprehensive care management across the care continuum.
• Promote optimal transitions to appropriate levels of care and timely discharge planning.
• Collaborate with the healthcare team and patient/caregiver to develop and implement care plans.
• Ensure clinical appropriateness and monitor LOS while coordinating resources

Skills

Nursing experience

Critical care experience (preferred)

1-year acute care case management

Experience with community resources

Education

Associate Degree in Nursing
