Job summary

Sentara Careplex Hospital in Hampton, VA is hiring an Inpatient Case Manager, RN to support the Emergency Department and health plan members. Shift: Days (8 hours). The role contributes to care coordination within the hospital's integrated delivery system.

The ICM, RN works with health-plan leadership to manage high-risk members, focus on transitions, utilization reduction, and population health objectives. Requires VA RN licensure, BLS, and case management certification within one year.

Qualifications

• Active Virginia RN license required; multi-state license acceptable.
• Case management certification required within one year.
• BLS certification required within 90 days of hire.
• 1 year nursing experience required; acute-care and case-management experience preferred.

Responsibilities

• Provide care management and quality outcomes across the healthcare continuum.
• Support hospital patients transitioning to appropriate level of care.
• Collaborate with the healthcare team and patient/caregiver to manage care and transitions.
• Develop and implement comprehensive care plans based on assessment and evaluation.
