Job summary

Sentara Williamsburg Regional Medical Center seeks an experienced Registered Nurse - Inpatient Case Manager to lead comprehensive care management for patients across the hospital continuum. The role focuses on transitioning patients to appropriate care levels while ensuring clinical appropriateness and length of stay optimization.

The nurse will develop integrated care plans, collaborate with the care team and family, and promote efficient use of services to support high-quality outcomes.

Qualifications

  • 12 months nursing experience required
  • BLS within 90 days of hire
  • Associates of nursing required; BSN preferred
  • Certification in Case Management within 1 year of eligibility

Responsibilities

  • Coordinate inpatient case management and facilitate patient transitions
  • Develop and implement comprehensive care plans based on assessments
  • Collaborate with health-care team and patient/caregiver to manage care across settings
  • Support discharge planning and resource utilization to ensure appropriate level of care

Education

Associate degree in Nursing
BSN preferred
Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification within 1 year

Overview

The Registered Nurse- Inpatient Case Manager is responsible and accountable for the provision and facilitation of comprehensive care management, services and quality outcomes for patients across the healthcare continuum, primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates, and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include physical, mental and psychosocial needs of the inpatient/caregiver.

Requirements
  • 12 months Nursing experience required; critical care experience preferred.
  • BLS within 90 days of hire.
  • Associates of nursing Required; BSN is preferred.
  • Case Management (ACM, CCM, CCCTM, CMAC or CMGT-BC) certification required within one year of eligibility.
  • 1-year acute care case management experience preferred.
