Responsibilities

• Facilitate and coordinate comprehensive care management for inpatients.
• Focus on patient transitions to appropriate level of care and LOS optimization.
• Collaborate with care team and patient/caregiver to develop care plans addressing physical, mental, and psychosocial needs.
• Support discharge planning and care continuity across the healthcare continuum.
