JMH is seeking an Inpatient RN Case Manager focused on patient advocacy and optimal resource utilization. The role coordinates care across the health-team spectrum, from admission through transition back to the community, under the full scope of nursing practice with emphasis on coordination and utilization management.

Education requires a California‑licensed RN with a BSN. Ideal candidates have 3+ years in medical/surgical or critical care and 2+ years in care coordination or case management,

Qualifications

• RN licensed in California required.
• Bachelor's degree in nursing required.
• 3+ years nursing experience in medical/surgical or critical care preferred.
• 2+ years in care coordination/case management preferred.
• Strong communication and teamwork skills.
• Knowledge of Medicare and Medi‑Cal guidelines.

Responsibilities

• Advocate for appropriate resource utilization and coordinate care across disciplines.
• Assess, plan, implement, monitor, and evaluate patient care during hospitalization.
• Facilitate hospital-to-community transitions with the interdisciplinary team.
