HealthPartners, Inc. is seeking an RN Inpatient Case Manager to coordinate inpatient care for hospitalized HealthPartners members. You will work with physicians, hospital staff, and social services to implement effective discharge plans and ensure timely treatment decisions.

The role emphasizes utilization management, patient advocacy, and collaboration to improve outcomes while controlling costs across the episode of care.

Qualifications

• Registered Nurse with Minnesota license; BSN preferred.
• Minimum 3 years experience in utilization review, case management or discharge planning.
• Excellent verbal, written and interpersonal skills.
• Ability to prioritize workload and work independently or as part of a team.
• Basic computer skills.

Responsibilities

• Provide on-site and/or telephonic inpatient case management and concurrent review.
• Monitor medical necessity, appropriateness and efficiency of care.
• Coordinate communication between patients, families, physicians, and care team.
• Coordinate transfer to in-network facilities when appropriate.
• Assist in monitoring LOS, readmissions, and cost savings performance.
• Maintain confidentiality and comply with policies and regulations.
