Job summary

HealthPartners is seeking an RN Inpatient Case Manager to coordinate inpatient care and perform concurrent review for HealthPartners members with medical/surgical needs. Qualifications

• RN with current unrestricted Minnesota license.
• Minimum 3 years of RN experience in clinical settings (utilization review, case management or discharge planning).
• Excellent verbal, written and interpersonal skills.
• Ability to work independently and in a team.
• Ability to navigate changing clinical environments.

Responsibilities

• Provide inpatient case management and concurrent review for HealthPartners members.
• Monitor medical necessity, appropriateness and efficiency of care.
• Participate in care coordination rounds and 1:1 meetings with physicians and staff.
• Maintain confidentiality and comply with policies.
• Coordinate discharge planning and transitions of care.
• Liaise with patients, families, providers, and social services.
