Job summary

HealthPartners is seeking an RN Inpatient Case Manager to coordinate inpatient care and concurrent reviews for hospitalized Medical/Surgical patients, with a focus on optimizing outcomes, timely treatment plans, and smooth transitions between levels of care.

You will collaborate with attending physicians, specialists and hospital staff to ensure effective plans, manage benefits and utilization, and contribute to cost-effective care while maintaining confidentiality and compliance with policies.

Qualifications

• RN license in Minnesota with no restrictions (BSN preferred).
• Minimum 3 years as RN in clinical setting with utilization review, case management, or discharge planning.
• Excellent verbal, written and interpersonal skills.
• Strong problem solving, organization and prioritization abilities.
• Ability to work independently and in a team with diverse staff and patients.
• Knowledge of transition planning and discharge processes.

Responsibilities

• Provide inpatient case management and concurrent review for HealthPartners members.
• Monitor medical necessity and efficiency of care with applicable guidelines.
• Participate in care coordination rounds and discussions with physicians and staff.
• Ensure timely discharge planning and transitions across care levels.
• Maintain confidentiality and comply with policies and procedures.
• Coordinate with providers, social services, and vendors for continuity of care.
• Assist in monitoring annual goals for LOS, readmissions, and cost savings.
• Stay current with case management and utilization guidelines and government requirements.
