Job summary

Responsibilities

• Oversee a daily caseload in inpatient units with diverse care needs.
• Attend interdisciplinary rounds and develop plans of care with patients and family.
• Communicate progress with medical teams and post-discharge care providers.
• Coordinate with post-acute referral sources for home health and rehab.
• Document and place orders for home health, DME, and nutritional needs in EMR.
