Job summary

Qualifications

• Minimum of 2 years in case management, utilization management, and acute care settings.
• RN or MSW in the state of Texas; LBSW accepted for candidates with 5+ years of demonstrated success.
• Certification in Case Management preferred, BLS preferred.

Responsibilities

• Collaborate with patient/family, physicians, and multidisciplinary teams to ensure progression through the continuum of care.
• Develop a plan of care from admission through discharge.
• Coordinate case management and discharge planning to optimize patient flow and length of stay.
• Identify and resolve barriers to effective patient care and advocate for patients related to treatment decisions.
