Qualifications

Responsibilities

• Conduct patient assessments and care coordination.
• Plan and manage discharge and post-acute care pathways.
• Coordinate with units for staffing and bed management.
• Handle calls Monday–Friday with rotating weekends and adapt to a fast-paced environment.
