Job summary

Advocate Health is seeking a Registered Nurse Case Manager for inpatient Sinai Medical Center in Milwaukee. You will work 40 hours weekly, typically 0800-1630, with occasional weekends and holidays, to manage discharge planning and care coordination on a cardiac/medical unit.

The role requires an RN license in the state, a preferred Bachelor’s in Nursing, and about two years of clinical nursing experience. Experience with EPIC and community resource knowledge is a plus.

Qualifications

• RN license in the state where practicing is required.
• Bachelor’s degree in nursing preferred.
• Minimum 2 years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and create timely care plans & interventions.
• Collaborate with patients, families, and the multidisciplinary team to meet goals and preferences.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Coordinate smooth transfers and handoffs across units and settings.
• Deliver case management services using community resources.
• Initiate referrals and document discharge planning and UM activities accurately.
• Perform concurrent/retrospective reviews using approved criteria.
• Educate staff on admission status, UM, care coordination, and regulatory requirements.
