Job summary

Sinai Hospital in Baltimore seeks an Inpatient Care Manager to coordinate discharge planning and care management for patients with complex needs. You will work closely with the clinical team to assess needs and plan safe transitions in the inpatient setting.

Qualifications

• Bachelor's degree or equivalent required
• BSN preferred; ADN required
• 3-5 years related experience in inpatient case management/discharge planning
• Maryland RN license or eligibility
• Case Management certification preferred within 3-5 years of hire
• Pay range provided: $50.00 at time of posting

Responsibilities

• Perform initial and ongoing care management assessment to determine discharge planning needs in the inpatient setting
• Create focused discharge plans for high-risk patients with identified needs
• Collaborate with clinical team to initiate the anticipated discharge plan
• Stay informed on regulatory matters and payer requirements related to care and discharge planning
