Job summary

Advocate Health's Sinai Medical Center in downtown Milwaukee is seeking an experienced Registered Nurse (RN) Case Manager for inpatient care management on a cardiac/medical unit. You will collaborate with social work, families, and the care team to assess needs, plan discharges, and support utilization management.

You'll conduct comprehensive assessments, develop timely care plans, and coordinate smooth transfers while ensuring regulatory compliance and high-quality patient care.

Qualifications

• RN license/Registration in the practice state
• Bachelor’s Degree in Nursing preferred
• Minimum 2 years of clinical nursing experience
• Strong inpatient background, with multi-disciplinary teams
• Care management/discharge planning experience, EPIC familiarity, knowledge of community resources

Responsibilities

• Conduct comprehensive assessments and create timely care plans
• Collaborate with patients, families and the multidisciplinary team to meet goals
• Provide continuity of care and discharge planning aligned with regulatory standards
• Coordinate transfers and handoffs across units and settings
• Deliver case management services using community resources
• Initiate referrals and document discharge planning and UM activities
• Perform concurrent/retrospective reviews using approved criteria
• Partner with Social Work to implement safe care plans
• Identify and remove discharge barriers; expedite care delivery and report UM outcomes
• Collaborate with managers, physicians and medical directors on status and compliance
• Complete UM activities and CMS notices within deadlines
• Build relationships with community agencies for ambulatory care coordination
• Educate staff on admission status, UM, care coordination and regulatory requirements
• Provide age-appropriate care per departmental standards
