Job summary

Aurora St. Luke’s Medical Center in Milwaukee, WI, is seeking a Registered Nurse (RN) – Inpatient Case Manager (1.0 FTE) to join our inpatient care team.

You will conduct comprehensive assessments, develop care plans, coordinate with families and multidisciplinary teams, and manage discharge planning while ensuring regulatory compliance.

This role emphasizes collaboration with social work, payer communications, and community resources to remove barriers to timely, quality patient care.

Qualifications

• RN license required in the state of practice.
• Bachelor’s degree in Nursing preferred.
• Minimum of 2 years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and create timely care plans and interventions.
• Coordinate with patients, families, and multidisciplinary team to meet goals.
• Provide continuity of care and discharge planning aligned with standards.
• Coordinate smooth transfers and handoffs across units and settings.
• Deliver case management services (healthcare access, finances, housing, family dynamics) using community resources.
• Perform UM activities and document CMS notices within required timeframes.
• Collaborate with Social Work and other staff to implement safe plans of care.
