Job summary

Advocate Health is seeking a Registered Nurse Case Manager for inpatient Sinai Medical Center in Milwaukee. You will conduct comprehensive assessments, plan care, and coordinate discharge planning to ensure safe transitions from hospital to home.

Work with patients, families, and the care team within a fast-paced inpatient setting. The role requires an RN license in the practicing state and at least 2 years of clinical nursing experience, with preference for inpatient and care management

Qualifications

• RN license issued by the state where you practice.
• Bachelor's Degree in Nursing preferred.
• Minimum 2 years of clinical nursing experience.
• Experience with EPIC and knowledge of community resources preferred.

Responsibilities

• Conduct comprehensive assessments and create timely care plans and interventions.
• Collaborate with patients, families, and the multidisciplinary team to meet goals and preferences.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Coordinate smooth transfers and handoffs across units and settings.
• Deliver case management services including health care access, finances, housing, and family dynamics.
