Job summary

Advocate Health in Milwaukee is seeking an RN Inpatient Case Manager to coordinate discharge planning and patient transitions within Aurora St. Luke’s Medical Center.

You will partner with patients, families, and the care team to assess needs, plan discharges, and support utilization management. The role requires an active RN license in the practicing state, preferred Bachelor’s in Nursing, and about 2 years of clinical nursing experience.

Qualifications

• Active Registered Nurse license issued by the practicing state.
• Preferred Bachelor's Degree in Nursing.
• Minimum of 2 years clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments and create timely care plans and interventions.
• Collaborate with patients, families, and multidisciplinary team to meet goals.
• Provide continuity of care and discharge planning aligned with regulatory standards.
• Coordinate smooth transfers and handoffs across units and settings.
• Deliver case management services including healthcare access, finances, housing, and family dynamics.
