Job summary

Encompass Health Corporation is seeking a Case Manager to coordinate care from admission to post-discharge, collaborating with an interdisciplinary team to optimize patient outcomes. You will manage resources and drive discharge planning in a leading national inpatient rehabilitation organization.

The role emphasizes clinical expertise, regulatory compliance, and compassionate patient care in a community-focused setting within Saint Louis, MO.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice (RN, SW, OT, PT, ST, Rehab Counseling).
• Active license if licensure is required by state.
• CCM® or ACM™ certification required OR within two years of start.

Responsibilities

• Guide treatment plans with interdisciplinary team based on patient needs and preferences.
• Coordinate tentative discharge plan and contingency plans with the team.
• Participate in planning and execution of patient discharge experiences.
• Monitor patient experience across quality, timeliness, service, payors and expectations.
• Facilitate weekly team conferences and coordinate plan modifications.
• Complete case management documentation and addendums.
• Maintain knowledge of regulations, policies, and department operations.
• Review case management reports and plan actions.

Education

Associate Degree in Nursing (RN) - BSN preferred

Bachelor's degree (for other licensed professionals)
