The role involves reviewing admission records, determining appropriate level of care, assessing discharge needs, and coordinating the discharge plan with the care team while fostering payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• Case Manager coordinates care for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization of hospital resources and admission level of care.
• Assesses discharge needs and coordinates discharge plan with the interdisciplinary team.
• Maintains and enhances payor relationships.
