Job summary

PAM Health Wiregrass Ranch is opening a brand-new rehabilitation hospital this fall and is seeking a Case Manager to coordinate interdisciplinary care from admission through discharge. You will assess patient needs, ensure appropriate levels of care, and collaborate with the care team to support financial and quality outcomes.

Eligible candidates may have licensure in the state, experience in inpatient settings, and a current BLS certification.

Qualifications

• Education and training: licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapy, physical therapy, registered nurse, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• Coordinate care across disciplines for designated patient population.
• Review inpatient admission records to ensure proper utilization of hospital resources and determination of admission for appropriate level of care.
• Assess discharge needs and coordinate appropriate discharge planning.
• Work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes.
• Maintain and enhance payor relationships.
