Job summary

HonorHealth is seeking a Physical Therapist to evaluate and treat patients with physical disabilities. You will develop and update care plans, collaborating with physicians and the care team to restore function and mobility.

The position requires delivering evidence-based therapies, documenting progress, and coordinating with case managers for discharge planning. This role supports rotating holidays and weekends across various HonorHealth centers in the Phoenix area.

Qualifications

• Licensed Physical Therapist in Arizona or eligible for licensure.
• Inpatient acute care setting experience preferred.
• Ability to work across multiple medical centers as needed.

Responsibilities

• Implements evaluation and treatment plans for patients with physical disabilities.
• Provides evidence-based therapy techniques and daily living activities training.
• Maintains accurate documentation of evaluations, plans, progress notes, and charges.
• Communicates with physicians, case managers, and care teams to coordinate care.
• Supervises therapy assistants and support staff, and updates treatment plans accordingly.
• Participates in interdisciplinary rounds and supports department process improvements.
• Contributes to patient education and discharge planning.
