Job summary

HonorHealth is seeking an Occupational Therapist to evaluate and provide evidence-based therapy to individuals with physical disabilities across multiple centers in the Phoenix area. The role emphasizes prevention of dysfunction, restoration of function, and collaborative care with patients and families.

Responsibilities include developing care plans, maintaining documentation, and coordinating with physicians, case managers, and the care team to support timely discharge and continuous quality

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate in Occupational Therapy preferred.
• 6 months Network Float/OT experience preferred.

Responsibilities

• Implements an individualized OT treatment plan with evidence-based techniques.
• Maintains accurate documentation in the patient’s medical record.
• Collaborates with interdisciplinary team to optimize discharge planning and outcomes.
• Supervises OT assistants and provides instruction to staff where appropriate.
