HonorHealth in Phoenix, AZ seeks an Occupational Therapist II to provide evaluation, treatment and oversight of OT services in Home Health. You will collaborate with patients and families to restore function and daily living skills, following physician guidelines and departmental policies.

Responsibilities include data gathering, plan development, and ongoing communication with care teams, with emphasis on evidence-based interventions and high-quality patient outcomes.

Qualifications

• Bachelor's degree in Occupational Therapy from an accredited university required.
• Master's degree in Occupational Therapy preferred by AOTA standards.
• 1 year Home Health experience or internship preferred; 3 years related experience preferred.
• Basic Life Support (BLS) certification required; OT license required.

Responsabilités

• Evaluate, administer, and oversee OT services for patients with functional limitations.
• Develop and update care plans with patient/family to restore function.
• Communicate with physicians, nurses, and case managers regarding patient status.
• Implement treatment plans with therapeutic exercises, mobility training, and ADLs.
• Document instructions for patients and families; ensure quality assurance activities.
