AccentCare Inc in Naperville, IL is seeking a Social Worker / MSW for the Inpatient Hospice program. This full-time, M-F position includes an on-call rotation, with salary range $65,000-$85,000 and on-site work.

Qualifications

• Master’s Degree from accredited school of Social Work; valid license/registration to practice in state of operation
• 1 year minimum post-graduate healthcare social work experience; 2+ years preferred
• Medicare/Medicaid/Insurance specialty preferred
• Experience or education in grief counseling preferred
• Discharge planning experience preferred

Responsibilities

• Assist core hospice team to understand social, spiritual, and emotional factors to plan care
• Participate with the interdisciplinary team in care plan development
• Assess social, spiritual, and emotional needs of patient and caregiver
• Prepare patient and family for end-of-life changes; educate on advance directives
• Utilize resources to support patient and family within illness limitations
• Support the bereavement program
• Provide discharge planning related to level-of-care changes
