AccentCare in St. Paul, MN is seeking a full-time Hospice Clinical Social Worker (MSW) to support patients and families across Minneapolis-St. Paul and surrounding areas.

In this role you will collaborate with the interdisciplinary team, assess social and emotional needs, and help plan care and transitions at end of life. Qualified candidates will hold an MSW, be licensed to practice in the state, and have experience in healthcare with preferred grief counseling and discharge planning.

Qualifications

• Master's degree in Social Work from an accredited program.
• Licensed to practice as a clinical social worker where required.
• Hospice experience preferred.

Responsibilities

• Assess social, spiritual, and emotional needs to establish a plan of care.
• Participate in interdisciplinary team discussions and plan reviews.
• Educate patients and families on advance directives and end-of-life planning.
• Support discharge planning and transitions between levels of care.
