Job summary

AccentCare, Inc. in St Paul, MN seeks a Hospice Social Worker (MSW) for a full-time, on-site role. Salary is $65,000–$85,000 with a Mon-Fri schedule and on-call rotation. This is an on-site opportunity within a collaborative hospice team.

You will support patients and families, participate in the interdisciplinary team, assist with discharge planning, and apply social, spiritual, and emotional care to improve quality of life for patients and caregivers.

Qualifications

• Master’s Degree from accredited school of Social Work or related field; Valid license/registration to practice in state of operation
• Medicare/Medicaid/Insurance specialty preferred
• At least one year of professional social work experience (post-graduate) in a healthcare setting; 2+ years preferred
• Experience or education in grief counseling preferred
• Discharge planning experience preferred

Responsibilities

• Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care
• Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients
• Assess the social, spiritual, and emotional needs to estimate patient and caregiver capacity to cope with daily living and terminal illness
• Prepare the patient and family to support changes and advance directives/advanced care planning
• Utilize resources to assist patient and family within illness limitations
• Support the bereavement program
• Provide discharge planning related to change of level-of-care or location-of-care
