Job summary

Sanctuary Hospice is seeking a dedicated Licensed Medical Social Worker (MSW) to provide compassionate social work services for patients and families. You will assess, counsel, and coordinate psychosocial support to help navigate end-of-life care with dignity.

This full-time, on-site role covers the Northern Cook, Lake and McHenry County area with a Monday–Friday schedule 8:00 am–4:30 pm. You will collaborate with an interdisciplinary team, advocate for patients, and guide families through care

Qualifications

• MSW degree from an accredited school
• Licensed as a Social Worker by the State Board
• Valid driver’s license and access to a dependable vehicle

Responsibilities

• Be a supportive guide and assess psychosocial status with RN Case Manager and team
• Evaluate medical needs, home situations, financial resources and community support
• Implement personalized social work plans and collaborate with Bereavement Coordinator
• Shape care plans, attend interdisciplinary meetings and address psychosocial stresses
• Provide counseling to patients and families with compassion
