Job summary

Atrium Health Hospice Union in Monroe is seeking a full-time Home Health Nurse to manage an assigned patient caseload in the home setting, ensuring timely care and accurate documentation according to the plan of care and physician orders.

You will collect data from home visits, telephonic contacts, and EMR systems, identify high-risk patients, and coordinate care with a multidisciplinary team to ensure high-quality outcomes and patient satisfaction.

Qualifications

• RN license required with current state licensure.
• BSN preferred.
• BLS certification required per policy.

Responsibilities

• Manage an assigned patient caseload in the home setting with timely care and documentation.
• Collect data from patient assessments, telephonic contacts, and EMR records.
• Identify high-risk patients and coordinate with the multidisciplinary team to mitigate risk.
• Provide care coordination to ensure quality outcomes and patient satisfaction.
• Supervise LPNs and HHAs involved in the care of the caseload.
