Job summary

Mosaic Life Care seeks an Inpatient Coder II to assign ICD-10-CM and ICD-10-PCS codes for acute care inpatient, rehab, swing bed and LTACH services. You will query providers, resolve documentation gaps, and ensure accurate coding in line with guidelines, while mentoring junior coders.

The role requires HIM education, 3+ years inpatient acute care coding experience, and active credentials (CCS/RHIA/RHIT). The position supports remote work for eligible states and emphasizes accuracy and education.

Qualifications

• Must have coding education with knowledge of medical terminology, pharmacology, anatomy, and query guidelines.
• Associate's Degree - Health Information Management / Medical Records - Required
• Bachelor's Degree - Health Information Management / Medical Records - Preferred
• CCS certification - Desired
• RHIA certification - Desired
• RHIT certification - Desired

Responsibilities

• Code complex diseases, procedures and diagnoses using ICD-10-CM/PCS per guidelines.
• Query providers following Official Query Guidelines to optimize documentation and severity of illness.
• Mentor and train coders.
• Ensure data accuracy for regulatory agencies by resolving coding and claim edits.
• Collaborate with clinical documentation, revenue integrity, and other departments to ensure accurate coding.
