Job summary

Trinity Health Ann Arbor is seeking a Clinical Pharmacist to provide medication preparation, distribution, and pharmaceutical care across a range of inpatient settings. You will contribute to high-quality, cost-effective, patient-focused care with a team of health professionals.

The role requires a Doctor of Pharmacy or equivalent, Michigan licensure eligibility, and willingness to work a rotating schedule in a 24/7 inpatient environment. Prior hospital experience is preferred.

Qualifications

• Registered or eligible Michigan pharmacist license.
• Ability to work in a fast-paced hospital environment.
• Strong communication and teamwork skills.

Responsibilities

• Interpret and evaluate medication orders for appropriate therapy.
• Monitor patient regimens for interactions, allergies, and contraindications.
• Individualize therapy based on renal function, age, weight and labs.
• Collaborate with physicians and nurses to optimize outcomes.
• Provide patient education and participate in antimicrobial stewardship.
• Document interventions and participate in quality assurance activities.
• Supervise pharmacy technicians and students as needed.
• Comply with USP 797/800 guidelines and regulatory requirements.

Education

Bachelor's degree in Pharmacy
