Job summary

Trinity Health in Ann Arbor is seeking a Clinical Pharmacist for the inpatient pharmacy team. The role involves medication preparation, distribution and pharmaceutical care across neonatal to geriatric populations, with a focus on safety and cost-effectiveness.

You will participate in rounds, therapeutic drug monitoring and education, while supporting CQI initiatives and interprofessional teaching. The position requires a license and hospital experience; rotating shifts apply.

Qualifications

• Bachelor's degree in Pharmacy required; Doctor of Pharmacy preferred.
• Michigan pharmacist license or eligibility required; BLS/ACLS as required.

Responsibilities

• Interpret and evaluate medication orders for appropriateness of therapy.
• Monitor patient regimens for interactions, allergies and contraindications.
• Individualize therapy based on renal function, age, weight and labs.
• Collaborate with physicians and staff to optimize medication use.
• Educate staff and patients on medications and adverse effects.
• Direct and supervise technicians and learners as assigned.
