Job summary

Trinity Health MI in Ann Arbor seeks a part-time Clinical Pharmacist for our Infusion Pharmacy to prepare, dispense, and monitor complex IV therapies. You will collaborate with the care team to ensure safe, effective patient care and optimize medication use in infusion services.

The role requires Michigan pharmacy licensure, hospital experience or residency preferred, and availability for day shifts with rotating hours as needed.

Qualifications

• Bachelor’s degree in Pharmacy required; Doctor of Pharmacy degree preferred.
• Licensed pharmacist in Michigan or eligible for licensure; ongoing CE requirements.
• Current BLS and/or ACLS certification as required by hospital policy.

Responsibilities

• Interprets and evaluates medication orders to ensure therapy appropriateness and dosing.
• Monitors patient med regimens for interactions, allergies, and adverse effects.
• Individualizes therapy based on patient factors such as renal function and labs.
• Collaborates with clinicians to optimize drug therapy and resolve unclear orders.
• Participates in rounds, education, antimicrobial stewardship, and drug information.
• Documents interventions and participates in quality and pharmacoeconomic reviews.
• Advises on safe, cost-effective medications and educates staff on care plans.
• Directs and verifies compounding of injectable meds per USP 797/800 guidelines.
