Job summary

Trinity Health is seeking a part-time Clinical Pharmacist to provide medication preparation, distribution, and clinical care across patient populations. You will participate in interdisciplinary education, CQI projects, and pharmacy residency instruction as part of a dynamic hospital team.

The role requires a PharmD or Bachelor’s in Pharmacy with Michigan licensure, BLS/ACLS as applicable, and prior hospital experience preferred. Rotating shifts and variable hours are expected.

Qualifications

• Required Bachelor's degree in Pharmacy or equivalent.
• Doctor of Pharmacy degree or equivalent clinical degree preferred.
• Previous hospital experience or residency strongly preferred.
• Michigan pharmacist license required with continuing education credits.

Responsibilities

• Interpret and evaluate medication orders for appropriate therapy including dose and route.
• Monitor patient regimens for allergies, interactions, contraindications and adverse effects.
• Individualize therapy based on renal function, age, weight and labs.
• Manage therapy across continuum of care through clinician communication; intervene when orders are unclear or inappropriate.
• Participate in patient rounds, therapeutic drug monitoring and antimicrobial stewardship.
• Educate physicians, staff and patients on pharmacotherapy and cost-effective care.
• Precept students and residents; participate in teaching programs as allowed.
• Maintain records for controlled substances and comply with USP 797/800 guidelines.
