Job summary

Trinity Health Ann Arbor is seeking a part-time Clinical Pharmacist to join the In-patient Pharmacy. The role focuses on medication preparation, distribution, and provision of pharmaceutical care for patients from neonatal to geriatric across a busy hospital setting.

Qualified candidates will have a required Bachelor's in Pharmacy (Doctor of Pharmacy preferred) and Michigan licensure, with hospital experience preferred.

Qualifications

• Required Bachelor's degree in Pharmacy; Doctor of Pharmacy degree or equivalent clinical degree preferred.
• Previous hospital experience or hospital residency strongly preferred.
• Licensed pharmacist in Michigan; continuing education as required by State Board of Pharmacy.

Responsibilities

• Interprets and evaluates medication orders for appropriateness of therapy, dosage form, and administration.
• Monitors patient regimens for allergies, interactions, contraindications, and adverse drug events.
• Individualizes therapy based on patient variables including renal function, age, weight and labs.
• Coordinates pharmaceutical services across units and communicates with physicians, nurses and staff.
• Participates in patient rounds, drug monitoring, education, and antimicrobial stewardship as applicable.
• Precepts and teaches students and residents; supports CQI initiatives.
