Job summary

Trinity Health MI invites a Clinical Pharmacist to join our hospital team in a part-time, rotating-shift role. The pharmacist will ensure safe and effective medication use, participate in rounds, and assist in education for staff and residents within the pharmacy department.

Responsibilities include evaluating orders, monitoring regimens for interactions, and optimizing therapy based on patient-specific factors, including PK and labs. BLS/ACLS and Michigan licensure are required.

Qualifications

• Required Doctor of Pharmacy degree; Bachelor’s degree acceptable with experience.
• Michigan Pharmacist license or eligibility; state board requirements followed.
• BLS or ACLS certification as required by hospital policy.

Responsibilities

• Interpret and evaluate medication orders for appropriate therapy and route.
• Monitor patient regimens for interactions, allergies, and adverse reactions.
• Individualize therapy based on renal function, age, weight, labs, and PK.
• Collaborate with physicians and nursing to optimize drug therapy across care continuum.
• Provide patient education, rounds, and antimicrobial stewardship activities.
