Job summary

Trinity Health Ann Arbor seeks a Clinical Pharmacist to provide medication preparation and distribution, and to participate in pharmaceutical care across neonatal to geriatric patients.

The role requires a Doctor of Pharmacy or equivalent, a current Michigan license or eligibility, and hospital experience preferred. You will work in a 24/7 inpatient pharmacy focused on high-quality, cost-effective care.

Qualifications

• Bachelor's in Pharmacy is required; Doctor of Pharmacy or equivalent is preferred.
• Previous hospital experience or residency strongly preferred.
• Licensed Michigan pharmacist or eligibility required; continuing education credits as per state law.
• Current BLS and/or ACLS certification as required by hospital policy.

Responsibilities

• Interpret and evaluate medication orders for therapy appropriateness, dosage, route, and timing.
• Monitor patient medication regimens for interactions, contraindications, and adverse drug events.
• Individualize therapy based on renal function, age, weight, labs, and pharmacokinetics.
• Manage drug therapy across the continuum of care through interprofessional communication.
• Participate in patient rounds, therapeutic drug monitoring, education, and antimicrobial stewardship.
• Document interventions and monitor therapy outcomes; participate in quality assurance and pharmacoeconomic analysis.
• Consult with physicians on safe, cost-effective medication plans.
• Educate staff and patients about pharmacotherapy goals and cost containment.
