Qualifications

• Bachelor's degree in nursing (preferred).
• Active RN license in NC/VA or compact.
• BLS certification required within 90 days.
• Case management certification within one year.

Responsibilities

• Provide evaluation and care management across the healthcare continuum.
• Support hospital patients needing comprehensive case management.
• Promote appropriate utilization and monitor health services.
• Develop care plans addressing physical, mental, and psychosocial needs.
